填寫此報名表，表示您希望參加10月5, 6 ,7日，三日會議。
包括10月5日上午之玩「殘」運工作坊、10月5日下午之障礙兒童論壇，以及10/6-7日正式之東亞障礙研究論壇會議。
其中，10月5日上午之玩「殘」運工作坊有資格與人數限制，錄取者將另行通知。
無論參加幾日會議均一價，午餐自理。
會議參加者之必要陪同者(限1名)免報名費，但請在報名表中註明，以便統計人數。
- 報名費用 (若未於指定日期前完成繳費，視同放棄報名)
- 早鳥價1000元 (8月15日前繳費完成)
- 一般報名1500元 (8月15日後- 9月20日前繳費者)
- 指定協辦單位之會員一般報名1200元(8月15日後- 9月20日前繳費者)
- 經濟困難者(含學生)1000元(請於9月20日前完成繳費)

Registration fee does not differ if you choose to participate only part of the conferences, lunches are not included, please manage your own lunches during the conference.
Conference participant's necessary campanion does not need to pay registration fee, however please fill up this registration form for arrange seating.
- Registration Fee
- Pay before August 15th, enjoy Early Bird Fee-NTD1000
- Pay between August 16th-September 15th- NTD1500
- For member of designated conference co-organizers who pay between August 16th-September 15th, NTD 1200
- Persons with Economy Difficulties(Students included), please pay before September 15th- NTD1000

For participants who are not located in Taiwan we accept creditcard or remittance payment.
